Vaginismus

What is Vaginismus?

Vaginismus is a feeling of vaginal tightness causing discomfort, burning, pain, penetration problems, or complete inability to have intercourse.

The involuntary tightening of the pelvic floor especially, the pubococcygeus (PC) muscle group, can cause vaginal stiffness, although many women may not aware that this can be a cause of her penetration or pain difficulties.

The common cause of Vaginismus is ongoing sexual pain and is also the primary female cause of sexless (unconsummated) marriages. Sexual pain in women can occur at any stage of life, even in the women having years of comfortable sex. While in the short-term women experiencing discomfort during sexual intercourse is not unusual, ongoing problems should be diagnosed and treated.

Vaginismus Common Symptoms:

  • Burning or stinging sensation with tightness during sex

  • Difficult or impossible penetration, entry pain, uncomfortable insertion of the penis

  • Ongoing sexual discomfort or pain because of Cancer, surgeries, childbirth, menopause, yeast/urinary infections, STDs, IC, hysterectomy, rape, or other issues

  • ongoing sexual pain with no apparent cause

  • Difficulty in inserting tampons

  • Difficulties during a pelvic/gynaecological exam

  • Spasms in legs, and lower back

  • halted breathing during intercourse attempt

  • Avoiding sex due to pain or failure

Vaginismus Non-Physical Causes

  • Fears – Fear of getting pregnant, fear of tissue damage, Fear or anticipation of intercourse pain, fear of not being completely physically healed following pelvic trauma, and a concern of getting the pelvic medical problem again.

  • Anxiety or Stress – General anxiety, performance pressures, negativity toward sex, guilt, emotional traumas, or other unhealthy sexual emotions

  • Partner issues – Losing control, Abuse, distrust, emotional detachment, fear of commitment, and anxiety about being vulnerable.

  • Traumatic events – Past emotional/sexual abuse, witness to violence or abuse, repressed memories

  • Childhood Experiences – Overly rigid parenting, unbalanced religious teaching, exposure to shocking sexual imagery, inadequate sex education

  • No cause – Sometimes cause is not identifiable, it may be physical or non-physical

Physical Causes for Vaginismus

  • Medical – Pelvic inflammatory disease, Urinary tract infections or urination problems, cysts, yeast infections, sexually transmitted disease, vaginal prolapse, endometriosis, genital or pelvic tumors, eczema, and psoriasis.

  • Childbirth – C-sections, Normal or difficult vaginal deliveries and complications, and miscarriages can cause pain

  • Age-related changes – Menopause, hormonal changes, vaginal atrophy, vaginal dryness or inadequate lubrication.

  • Temporary discomfort – Insufficient foreplay and inadequate vaginal lubrication can cause temporary pain or discomfort.

  • Pelvic trauma – Pelvic trauma, pelvic surgery or any difficult pelvic examinations

  • Abuse –Rape, sexual or physical abuse or assault and Physical attack,

  • Medications – Side-effects through certain medication or drugs may cause pelvic pain

Vaginal muscles automatically tighten, reacting to the anticipation of pain, trying to protect itself from harm. Depending upon the severity of this tightened state Sex becomes uncomfortable or painful, and entry may be more difficult or

impossible. Body experience increased pain more on an ongoing basis that entrenches this response and creates a vaginismus cycle of pain. Having physical events like inadequate foreplay, lubrication or non-physical emotions and general anxiety can trigger vaginismus, it is important to understand that there is no fault of a woman in vaginismus. When vaginismus gets triggered, resulting in the occurrence of involuntary muscle tightness with no conscious direction. A woman neither can direct her body to get it to tighten nor can make it stop. Women with vaginismus may initially be sexually responsive and deeply desire to make love but over time this desire may reduce due to pain and feelings of failure and discouragement. It is extremely frustrating to be unable to physically engage in satisfying sexual intercourse.

The symptoms of vaginismus are, the anticipation of pain, emotional anxieties, or unhealthy sexual messages can contribute to and reinforce. Often, but not at all time, there are deep-seated underlying negative feelings of nervousness associated with vaginal penetration. Emotional triggers can be responsible for vaginismus but the symptoms of this are not apparent, it requires explanation. It is important that effective treatment processes include addressing any emotional triggers so a full pain-free and pleasurable sexual relationship can be enjoyed upon resolution.

Vaginismus can lead muscles around the vagina to tighten involuntarily. This can cause some pain and discomfort. Many women will experience it at some point in their life whether they have had sex or not.

When the genital area is touched, usually, Vaginismus occurs. For example, it can be before sexual intercourse, attempting to insert a tampon, or during a gynaecological examination.
